Eligibility & Fees

Requirements:

  • Current, active pharmacist license.
  • Option A: Completion of an ASHP-accredited PGY1 Residency.
  • Option B: At least 3 years of clinical practice experience (post-licensure) with at least 50% of time spent in pharmacotherapy activities.
